Gus Halvorsen has kept the same fleet of twenty-two school buses running for nineteen years, and he can name a failing wheel bearing by its hum before he ever rolls under the chassis. His own stomach is harder to read. Since March, lunch has sat in it like a fist, and the roll of chalky tablets in his truck's cup holder is down to three. Stuck to the dash beside it is a sticky note in his sister's handwriting: Pyloricil Ortho Molecular.

She's a hospice nurse in Duluth, and the text she sent with it said only: "Ask Dr. Patel about this one." The note has been on the dash for a week. Gus hasn't looked it up. He isn't sure he wants to be a guy who takes capsules for his stomach.

Pyloricil is a practitioner-only capsule from Ortho Molecular Products that combines four ingredients aimed at the stomach: 250 mg mastic gum extract, 125 mg bismuth citrate, 150 mg berberine hydrochloride hydrate, and 37.5 mg zinc carnosine (supplying 8.5 mg of zinc) in each capsule. Ortho Molecular formulates it to promote healthy microbial balance in the GI tract, help maintain gastrointestinal comfort, and soothe the stomach and GI tract. The label dose is one capsule twice a day, so a 60-count bottle lasts 30 days. Do not take it if you are pregnant or nursing.

What is Pyloricil Ortho Molecular?

Pyloricil is a stomach-focused formula. That sounds obvious, but it matters, because most of what gets sold as "gut support" is built for the lower intestine: probiotics, fiber, enzymes. Pyloricil works higher up, where food first meets acid and where the stomach lining has to tolerate that acid every day.

Ortho Molecular describes the formula as supporting the health of the stomach lining and GI tract with ingredients that promote microbial balance. The company files it under two categories, Gastrointestinal Health and Immune Health, and its product overview explains the second one this way: the body's immune system is a complex defense system, and healthy microbial balance in the gut helps it function well.

Each ingredient has a different job. Two of them (bismuth citrate and zinc carnosine) are minerals that act mostly on the surface of the stomach lining. One (mastic gum) is a tree resin with a long history of stomach use around the Mediterranean. The fourth (berberine) is a plant alkaloid that Ortho Molecular includes for microbial and inflammatory balance. The design choice is breadth: four different mechanisms in one capsule, instead of a high dose of one thing.

Ortho Molecular sells only through licensed health care practitioners, which is why Pyloricil requires clinic access at Covenant Health Products. That gate is not a sales gimmick. A stomach complaint can be a dozen different things, and some of them need a doctor, not a supplement. Having a practitioner in the loop is the point.

What's in Pyloricil, and how do the ingredients work?

Here is the full Supplement Facts panel from the Pyloricil Ortho Molecular label, per one-capsule serving, followed by what each ingredient actually does in the stomach.

IngredientPer capsulePer day (2 capsules)
Mastic Gum Extract (Pistacia lentiscus) (resin)250 mg500 mg
Berberine Hydrochloride Hydrate150 mg300 mg
Bismuth Citrate USP125 mg250 mg
Zinc Carnosine37.5 mg75 mg

Other ingredients: natural vegetable capsule, microcrystalline cellulose, stearic acid, and magnesium stearate. Free of gluten, yeast, and artificial colors or flavors.

Mastic gum (250 mg)

Mastic is the resin that weeps from cuts in the bark of Pistacia lentiscus, an evergreen shrub native to the Mediterranean Basin. On the Greek island of Chios it has been harvested for well over two thousand years, and Ortho Molecular notes its historical use in supporting stomach health. Most of mastic's activity in lab research has been traced to its triterpenic acids, a class of resin compounds concentrated in the acidic fraction of the gum. Ortho Molecular states that mastic has been shown to promote healthy microbial balance in the GI tract.

One detail worth knowing: the two-week double-blind trial Ortho Molecular cites on its product sheet gave participants 1 gram of mastic a day. Two capsules of Pyloricil provide 500 mg. That doesn't make the formula weak, since mastic here is one of four ingredients rather than the whole show, but it does mean you shouldn't read that study as a direct measure of what Pyloricil delivers.

Bismuth citrate (125 mg)

Bismuth is a heavy mineral, and very little of it gets absorbed. Reviews of bismuth pharmacology put absorption under 1% of the amount swallowed. That's the point. It stays in the stomach, where it tends to form a protective coating over the mucosa and has been shown to increase mucus glycoprotein secretion, the slick layer that sits between stomach acid and the cells of the lining. Ortho Molecular includes bismuth citrate to soothe the gastric and mucosal linings and to promote microbial balance in the GI tract.

The manufacturer also makes a point that most product pages skip: bacteria commonly develop resistance to many balancing agents, but they are unlikely to develop resistance to bismuth citrate. Bismuth's effects are physical and chemical rather than tied to one bacterial target, which is a large part of why it has stayed in use for well over a century.

Berberine hydrochloride hydrate (150 mg)

Berberine is a bright yellow alkaloid found in the roots and bark of Oregon grape, barberry, and goldenseal, with a long history in both Ayurvedic and Chinese medicine. Ortho Molecular includes it to promote healthy microbial balance in the GI tract and to help maintain normal inflammatory balance throughout the body.

A note on the label, because readers comparing sources will run into it: Ortho Molecular's older product copy describes this ingredient as berberine sulfate. The current Pyloricil Ortho Molecular label, on both the manufacturer's product sheet and the bottle, lists berberine hydrochloride hydrate at 150 mg. The label is what's in the capsule. Berberine also has the most going on pharmacologically of the four ingredients, which is why it gets its own paragraph in the safety section below.

Zinc carnosine (37.5 mg)

Zinc carnosine is not zinc plus carnosine in the same capsule. It is a single compound in which zinc and the dipeptide L-carnosine are chemically bonded, developed by Japanese researchers and approved in Japan since 1994 for its use in stomach health. Ortho Molecular notes that zinc's effects are enhanced significantly when paired with carnosine in this form.

The best-known human data comes from a 2007 study published in Gut. Ten healthy volunteers took a common anti-inflammatory drug known to make the gut lining leakier. In the placebo phase, gut permeability tripled. With zinc carnosine, it didn't increase at all. Lab work in the same paper showed the compound encouraged gut-lining cells to migrate and multiply, which is how the lining renews itself. The authors' conclusion was that zinc carnosine stabilizes the gut mucosa. It's a small study, and the people in it were healthy, but the mechanism is clear and it's the reason this ingredient shows up in so many stomach formulas.

Who is Pyloricil for, and who should not take it?

Pyloricil makes the most sense for adults whose practitioner wants several stomach-focused ingredients working at once. Some examples of where it tends to come up:

  • A 50-year-old whose stomach feels unsettled after meals, who has already been evaluated by a doctor, and whose practitioner wants to add nutritional support for the stomach lining.
  • Someone working with an integrative practitioner on a broader GI plan, where Pyloricil covers the upper GI while a probiotic like Ortho Biotic handles the lower gut.
  • People who have tried zinc carnosine or mastic on its own and whose practitioner now wants the combination instead.
  • Anyone who would rather take one formula twice a day than track four separate bottles.

Who should not take Pyloricil

The caution on the Pyloricil Ortho Molecular label is direct: Do not consume this product if you are pregnant or nursing. Consult your physician for further information. That's a "do not," not a "talk to your doctor first."

Beyond the label, a few groups should get a practitioner's sign-off before starting: people taking prescription medications (especially anything for blood sugar, see the safety section), people already taking a separate zinc supplement or a multivitamin with zinc, and anyone whose stomach symptoms are new, getting worse, or showing up alongside weight loss, vomiting, trouble swallowing, or black stools. Those last signs need a doctor's visit, not a supplement.

How does Pyloricil Ortho Molecular compare to PyloGuard and single-ingredient options?

Practitioners usually weigh Pyloricil against two alternatives: a probiotic built for the stomach, like PyloGuard from Microbiome Labs, or a single ingredient such as zinc carnosine on its own. They take very different routes to similar goals.

Where it winsBreadth: lining support and microbial balance in one formulaSimplicity and cost; once a day; no mineral or alkaloid loadLowest cost; fewest variables; easy to trial

To be straight about it: if you're on several prescriptions, PyloGuard or zinc carnosine alone may be the better first step, simply because neither contains berberine. And PyloGuard costs about $15 less a month at one capsule a day. Pyloricil earns its place when a practitioner specifically wants mastic, bismuth, berberine, and zinc carnosine together, which is a different strategy than adding a single probiotic strain. For a related look at how practitioners think about microbial balance products, our post on GI Microb-X by Designs for Health covers a botanical formula with a different focus.

How much Pyloricil should you take, and when?

The Pyloricil Ortho Molecular label calls for 1 capsule two times per day, or as recommended by your health care professional. A 60-capsule bottle is a 30-day supply at that dose.

Ortho Molecular's label does not say whether to take Pyloricil with or without food, and it doesn't give a set duration. The simplest way to follow a twice-daily label is to space the capsules about twelve hours apart, one in the morning and one in the evening, so the stomach has some of each ingredient present through the day. If capsules on an empty stomach bother you, ask your practitioner whether taking them with meals is fine for your situation.

On timing expectations, the label is silent. The mastic study Ortho Molecular references ran two weeks, and a full bottle runs 30 days. A practical approach is to finish one bottle, keep notes on how your stomach feels after meals, and review with your practitioner before deciding on a second. Don't increase the dose to speed things up. The label amount is the label amount.

What won't Pyloricil do?

This is the section most supplement pages leave out, so here it is plainly.

It isn't a substitute for medical care. Pyloricil's name makes some people assume it's built to replace a doctor's treatment for H. pylori. It isn't, and Ortho Molecular doesn't claim that. If you've tested positive for a stomach infection, follow the treatment your doctor prescribes. If you take a prescribed acid-reducing medication, don't stop it to try Pyloricil.

It isn't for lower-gut complaints. If your problem is bloating low in the belly, irregularity, or changes in your bowel habits, Pyloricil isn't aimed there. A probiotic, fiber, or a formula like GI Revive is usually a better match.

It won't override what's on your plate. Big late meals, a lot of alcohol, and long stretches between meals all make a stomach work harder. Pyloricil supports the lining; it doesn't cancel out habits that keep irritating it.

It probably isn't worth it if your discomfort is occasional. Two capsules a day at about $68 a month is a real commitment. For a once-a-month rough stomach after a heavy dinner, it's more product than the problem needs.

Is Pyloricil safe? Side effects and interactions to know

The Pyloricil Ortho Molecular label warning: Do not consume this product if you are pregnant or nursing. Consult your physician for further information.

Ortho Molecular's label doesn't list side effects, so what follows comes from what's known about the individual ingredients. None of it replaces a conversation with your own practitioner or pharmacist.

Bismuth can turn your stool dark or black. It's a harmless chemical reaction in the gut, but it matters for one reason almost nobody mentions: black stool is also a warning sign of bleeding in the upper GI tract. If you're taking Pyloricil and your doctor asks about stool color, tell them you're taking a bismuth product. And if you notice black stool along with dizziness, weakness, or stomach pain, get checked rather than assuming it's the bismuth.

Berberine can interact with medications. In a human study, two weeks of berberine at 900 mg a day reduced the activity of several liver enzymes (CYP2D6, CYP2C9, and CYP3A4) that clear many common drugs. Pyloricil provides 300 mg a day, a third of that amount, but the direction of the effect is the reason to check. Berberine is also studied for its effects on blood sugar, so anyone taking medication for blood sugar should talk with their prescriber before starting. Mild digestive effects such as cramping or constipation have been reported with berberine.

Watch your total zinc. Two capsules supply 17 mg of zinc. The NIH Office of Dietary Supplements sets the adult upper limit at 40 mg a day from all sources, and notes that long-term intake above that can lower copper levels. If your multivitamin or another supplement adds 20 mg or more, you're getting close. Add up your labels.

As with any supplement, consult your healthcare provider before use, especially if you are pregnant, nursing, or taking medications.

Is Pyloricil Ortho Molecular worth it?

For the right person, yes. If your practitioner wants combined support for the stomach lining and microbial balance, and you're not pregnant, not nursing, and have cleared any medication questions, Pyloricil puts four well-chosen ingredients into a simple twice-daily routine. The zinc carnosine and bismuth work on the lining itself, which is what sets it apart from the probiotic-first products that dominate gut shelves.

It's not the right pick if your symptoms haven't been evaluated, if they sit in the lower gut, or if you take medications that make berberine a question mark. The honest trade-off is cost and complexity: you're paying for breadth, and some people would do just as well starting with one ingredient.

Gus, for his part, finally looked up the name on his sister's note the night before his appointment, then peeled it off the dash and tucked it in his wallet to show Dr. Patel.

FAQs

Can I take Pyloricil while pregnant or breastfeeding?
No. The Pyloricil Ortho Molecular label says: "Do not consume this product if you are pregnant or nursing. Consult your physician for further information." If you are pregnant, planning to be, or nursing, talk with your physician about other options.
Should I take Pyloricil with food?
The label doesn't say. Ortho Molecular's directions are one capsule two times per day, or as recommended by your health care professional, with no food instruction. If capsules on an empty stomach bother you, ask your practitioner whether taking them with meals is fine for you.
Does Pyloricil turn your stool black?
It can. Bismuth citrate commonly darkens stool, and the change is harmless on its own. Because black stool can also signal bleeding in the upper GI tract, tell your doctor you take a bismuth product, and get checked if dark stool comes with dizziness, weakness, or stomach pain.
Can I take Pyloricil with a multivitamin?
Often, but check the zinc. Two Pyloricil capsules provide 17 mg of zinc, and the NIH adult upper limit is 40 mg a day from all sources. If your multivitamin adds 20 mg or more, review the total with your practitioner before combining them.
How long should I take Pyloricil?
Ortho Molecular doesn't set a duration on the label. A 60-capsule bottle lasts 30 days at the label dose, which gives you and your practitioner a reasonable window to decide whether to continue.
Do I need a prescription to buy Pyloricil?
No prescription is needed. Like every Ortho Molecular product, Pyloricil is sold only through licensed health care practitioners. At Covenant Health Products, you can buy it after joining the clinic, which is free.
